Abstract Details

Purpose:

To determine the efficacy of cataract extraction with implantation of toric intraocular lenses (tIOL) after penetrating keratoplasty to correct high astigmatism.

Setting:

Hospital Pedro Hispano, Oporto, Portugal.

Methods:

Six eyes of 6 patients (mean, 61 years; range, 52-79 years) who had substantial astigmatism after penetrating corneal grafts underwent phacoemulsification with tIOL implantation. Uncorrected visual acuity (UCVA), best spectacle-corrected visual acuity (BSCVA), refraction, endothelial cell count (ECC) were measured before and 4 months after surgery. Anterior corneal topography using a Scheimpflug videophotography system and patient satisfaction were also assessed.

Results:

The mean +/- standard deviation of the preoperative refractive cylinder was -4,42+/-1,36 diopters (D; range, -2,0 to -6,0 D) which was reduced to -0,46+/-0,25 D at 6 months. Spherical equivalent was reduced from -4,38+/-4,27 D before surgery to -0,39+/-0,44 at the final follow-up. There was a gain of uncorrected and best-corrected visual acuities from 0,08+/-0,25 and 0,30+/-0,25 to 0,52+/-0,18, and 0,60+/-1,67, respectively. The percent reduction in refractive astigmatism was 89,6%. The mean endothelial cell loss with phacoemulsification was 7,5% at the last follow-up.

Conclusions:

Toric intraocular lens implantation after penetrating keratoplasty was effective for reduction of refractive astigmatism. All patients were very satisfied with post-operative visual results.
