Purpose:
To demonstrate the initial results of an intrastromal arcuate keratotomy (ISAK) performed with a femtosecond laser (150 kHz IntraLase iFS System,IntraLase Corp., Abbott Laboratories Inc., Illinois, USA)

Methods:
In this prospective single center study, patients with astigmatism (naturally occurring or post cataract surgery) were treated with the iFs-laser performing arcuate cuts completely placed within the corneal stroma on the steep axis. Patients were followed for six months after surgery.
Results:
Up to date 16 were treated with this method. No perforations occurred with all incisions being placed at the planned location. They were barely visible by slit-lamp examination at the first postoperative day, but could be detected with the OCT (Visante, CZM, Germany). After a mean follow up period of 6 month, mean subjective cylinder improved significantly from 1.41 ± 0.66 D preoperatively to 0.33 ± 0.42 D (p=...). Also mean topographic astigmatism could be reduced significantly from 1.50 ± 0.47 D preoperatively to 0.63 ± 0.34 D 6 months post ISAK (p=...). There was a high stability of the refractive error and corneal topography over time. Endothelial cell count was found to be unchanged as compared with preoperative values. Patient´s satisfaction was very high.
Conclusions:
The iFS-laser allows the creation of precise, purely intrastromal incision patterns that are not readily achievable by standard diamond blade techniques. These preliminary outcomes indicate an excellent safety profile, the possibility of highly precise pattern placement and very rapid recovery and stability of vision.
