Purpose:
To present a case of advanced keratoconus successfully treated with corneal cross-linking in association with the application of microwaves
Setting:

Methods:
Our patient is a 29-nine- year old female who was diagnosed of keratoconus in her left eye. She presented visual acuity of 0.02 ( without correction ) and 0.3 ( with correction ). The correction was myopia of / -9 Dsph and -12 D cyl ax 5/. We realized a test of visual acuity with rigid contact lenses reaching 0.6 ( without correction ). In the examination we saw signs of keratoconus with central corneal thinning. The corneal topography showed: central pachymetry of 342 microns, K max. of 56.6 dioptries and K min. of 48.9 dioptries ( Sim K 7.7 dioptries ). The patient rejected the realization of deep lamellar keratoplasty, so keeping in the mind the examination, the clinical sintoms and the progression we decided to realize cross-linking associated with application of microwaves.
Results:
One year after the procedure our patient showed visual acuity of 0.75 ( without correction ) and it is not getting better with correction. The last corneal topography is showing: central corneal pachymetry of 344 microns, K max. of 46.7 dioptries and K min. of 42.5 dioptries ( Sim K 4.2 dioptries ).
Conclusions:
The corneal cross-linking associated with the application of microwaves is a viable treatment option for progressive keratoconus. This treatment can detain the progression of keratoconus and can improve the appearance of the corneal curvature.
