To observe the clinical and refractive outcomes of Topolyzer-guided subjective preference (TSP) LASIK procedure in myopes

Abstract Details

Purpose:

To determine the outcome of LASIK procedure where astigmatic data of TSP is used instead of direct measurement from the topolyzer in the topoguided module on the Alcon EX 500 machine

Setting:

All procedures were performed by a single surgeon in the Refractive Suite at the Aga Khan University Hospitals, Karachi Pakistan.

Methods:

Prospective consecutive data collected on 538 eyes treated on FS 200 and EX500 from November 2016 to February 2019 Topolyzer guided Subjective Preference or TSP involves performing refraction using corneal astigmatism data from the topolyzer and using that as a baseline for subjective refraction. The end point is the refraction that patient subjectively finds the best. Patient is then given glasses to try for at least a week to determine the subjective quality of the refractive error.Subject to the patient's satisfaction, this astigmatic information is used in topoguided module for LASIK

Results:

538 consecutive treated eyes were studied from Nov2016–Feb2019. Age ranged from18-45 years; 362 were women and 166 men. The range of treated sphere was from -0.5 to -9.5,with maximum cylindrical being -4.75. All patients reported same or better vision post operatively. 98 % of the eyes were 20/20 or better post operatively, out of which 77.4% were 20/15 and 5.98% eyes were 20/10. The results were maintained on subsequent post-operative follow ups at 1 week, 2 weeks, 1 month and 3 months

Conclusions:

Literature review suggests Topoguided refraction has showed improved refractive outcome. Topoguided technique relies completely on topolyzer data. The theory behind Topolyzer guided Subjective Preference (TSP) is that it uses global refractive change including patient’s visual processing in choosing the best possible refractive correction
