Laser top up to produce monovision on multifocal IOL patients

Abstract Details

Purpose:

To describe 8 cases of patients over the age of 50 , who had bilateral multifocal IOL as Refractive Lens Exchange , with good distance vision and very low refractive errors , but they did not achieve satisfactory near vision ,thus they had top up Lasek treatment in the non-dominant eye to produce mono-vision , in order to achieve satisfactory near vision.

Setting:

Optimax and Ultralase Refractive Surgeries clinics in UK .

Methods:

There were 8 patients , 6 women and 2 men aged between 50-62 , who all had bilateral Multifocal IOL . They achieved distance vision between 6/6 to 6/9 with minimal refractive errors ,but near vision was restricted to N12-N18 , Those  patients were not satisfied with their near vision , All these patients had a trial of 2-3 months of monovision contact lenses on the non dominant eye , they all were satisfied with monovision contact lens vision . Thus top up Lasek hypermetropia treatment was performed to aim for -1,50 myopia on the non-dominant eye.

Results:

All these patients achieved satisfactory near vision recorded at 3 months and 12 months after laser top up . 4 patients achieved -1.50 D and 3 patients achieved -1.75 D and 1 patient achieved -2,00 D in the Laser treated non-dominant eye , the near vision for all these 8 patients on the non-dominant eye was satisfactory with 5 patients achieving N5 and 3 patients achieving N6 vision . There is one patient had recurrent corneal abrasion which completely settled down after 6 months from the laser top up. There was no refractive regression  for 12 months post laser .

Conclusions:

We conclude that Iasek top up on the non-dominant eye to achieve myopic monovision can be a good solution , in patients who had Multifocal Refractive IOL replacement surgeries , but are not satisfied with their near vision. All these patients should have careful and lengthy  monovision contact lenses trial before the Lasek top up.
