Purpose:
To report a case of blast infiltration of the Optic nerve associated with a central retinal artery and vein occlusion in a young man with lymphoblastic leukemia
Setting:
Departement of Ophtalmology B,chu Avicenne,Rabat,Morocco
Methods:
We present the case of a 27 year old man, having as main antecedent a LAL discovered 8 months ago, treated and in complete remission at the time of the ophthalmological consultation, consults urgently for a brutal decrease of visual acuity of the left eye associated with left periorbital pain.
Results:
The visual acuity is encrypted at 10/10 in the right eye with a lack of light perception in the left eye. Examination of the pupillary reflex finds a left relative afferent pupillary deficit. The right fundus examination is normal. The left fundus shows a whitish infiltration of the optic nerve associated with a mixed occlusion of the central retinal artery and vein. , the patient is referred to hematology service; hematological recurrence of the LAL is confirmed. General and intrathecal chemotherapy is initiated urgently. Two months later, the patient presents a left retinal sequellae of mixed arteriovenous occlusion.
Conclusions:
Blast infiltration of the optic nerve during ALL is rare, severe and can be associated with central retinal artery and vein occlusions.
