Gore-Tex suture assisted scleral fixation of intraocular lenses in the pediatric age group

Abstract Details

Purpose:

To evaluate the clinical outcomes and safety profile of scleral fixated intraocular lenses (IOLs) using Gore-Tex suture in the paediatric age group

Setting:

Pediatric ophthalmology clinic, Guru Nanak Eye Centre, Maulana Azad Medical College, New Delhi

Methods:

The study was a retrospective, interventional case series including 24 eyes of patients less than 14years of age who underwent ab-externo scleral fixation of Bausch and Lomb Akreos AO60 IOL using 8-0 Gore-Tex suture. The main outcomes measured were change in visual acuity, post-operative IOL centration (using slit-lamp), IOL tilt (using ultrasound biomicroscopy) and occurrence of intraoperative and post-operative complications.

Results:

A total of 24 eyes with a mean age of 10.21±5.73 years were included in the study. The mean follow-up period was 391.4 days. The mean logMAR UCVA improved from 1.43±0.48 preoperatively to 0.80±0.36 and BCVA improved from 0.93±0.44 preoperatively to 0.37±0.26. All the IOLs were found to be well centred on slitlamp examination.  The average horizontal and vertical IOL tilt on UBM was 211.50±483.67 μm and 112.00±95.51 μm respectively. Vitreous hemorrhage was noted in 2 eyes, which resolved spontaneously. Other potential complications like retinal detachment, glaucoma and suture breakage were not seen.

Conclusions:

Scleral fixation of IOLs using Gore-Tex suture is an effective method for visual rehabilitation in eyes with inadequate capsular support and markedly subluxated lenses. Gore-Tex suture is an excellent alternative to prolene sutures and is free from complications like suture breakage and knot erosion. Scleral fixation of IOL with Gore-Tex suture in pediatric eyes resulted in marked improvement of visual acuity with no sight threatening complications and provided excellent IOL centration.
