Abstract Details

Purpose:

The aim of the presented study is to show the clinical case, the diagnostic path, clinical traps and treatment of the patient with acute corneal decompensation after removal of floating iris cyst.

Setting:

Hospitalization of patient, treatment and diagnostics was performed in Department of Ophthalmology, Professor K. Gibinski University Clinical Center of the Medical University of Silesia, Katowice,  Poland.

Methods:

41 years old patient with suspected intraocular iris tumor was admitted to Department of Ophthalmology of Medical University of Silesia in Katowice for diagnostic and treatment. Patient connected onset of his ophthalmological complaints - eye pain and conjunctival redness with former trauma. Extended diagnostics was carried out including: static and kinetic visual field examination, optic coherence ocular tomography of anterior segment of the eyes, Confoscan microscopy, Ocular Ultrasound, Ultrasound Biomicroscopy and orbital MRI.

Results:

Performed studies confirmed the presence of a mobile lesion in the anterior chamber of the eye - changing the location depending on the patient's position and pupil size, and therefore decision to carry out the procedure of removal was done. After surgery we observed sudden increase of IOP and consequently decompensation of cornea and unsealing of the cornea wound. Patient required intensive local and general treatment and after 1 week we achieved normalisation of IOP and cornea structure. The postoperative histopathological examination result showed that mobile lesion was an iris cyst.

Conclusions:

The surgical treatment resulted in the removal of a suspicious eye lesion and subsequent pharmacological treatment led to withdrawal of corneal oedema and stabilization of intraocular pressure.
