Abstract Details

Purpose:

Demodex is a mite of the commensal flora and the palpebral free edge. the demodexfolliculorum is found in the eyelashes and brevis in the meibomian glands. The aim of this work is to analyze the effects of tea tree oil against the demodex

Setting:

Adult Ophthalmology department, Ibn Rochd University Hospital , Faculty of medicine and pharmacy, Hassan II University, Casablanca.

Methods:

A 65-year-old diabetic woman consulted for chronic blepharitis. Examination found an anterior blepharitis with collars at the base of the eyelashes in both eyes and a decreased BUT in both eyes. Treatment consisted of eyelid hygiene using micellar foam, lubricant without preservatives, tea tree oil diluted with mineral oil at different concentrations with weekly scrubs with 50% of tea tree oil in clinic and daily scrubs with 10% concentration at home. Eyelid hygiene and care was explained and monitored closely every week for 4 weeks.

Results:

Evolution was marked by regression of crusts and blepharitis. The role of demodex in the pathology of blepharitis remains a subject of debate. Alteration of ocular surface is related to the demodex through dry eye syndrome and blepharitis. Tea tree oil was used for the first time in Australia for skin wounds and infections. The elimination of the demodex by tea tree oil has been proven efficient in vitro and in vivo. This oil consists of two essential components: 1,8-cineole and 4-ol terpinen which are inhibitors of acetylcholinesterase with an acaricide effect stimulating the migration and destruction of the demodex.

Conclusions:

Blepharitis caused by demodex remains a subject of debate. Demodex folliculorum should be mentioned when faced with any chronic blepharitis. Hygiene, exfoliation and proper care of the eyelids and eyelashes with tea tree oil has shown its efficiency in combination with clear instructions and compliance.
