Abstract Details
Purpose:
We present a case of Abiotrophia defectiva (A. defectiva) endophthalmitis following routine cataract surgery. To our knowledge this is the first reported case in the UK and fourth globally.
Setting:
A. defectiva is a fastidious organism, its main virulence factor being through adhesion to fibronectin. No specific antibiotic resistances have been reported. Nowadays modern tools are available to aid identification in culture-negative cases, though the main challenge remains clinical suspicion of A. defectiva as the potential causative agent.
Methods:
We have assessed a case of exogenous endophthalmitis 2 days after routine cataract surgery. A vitreous tap and intravitreal antibiotic injections were performed. A. defectiva was identified using the MALDI-TOF MS® technique (Bruker Daltonik, Germany).
Results:
Over the next 3 months clinical improvement could be observed, the vision reaching a best-corrected visual acuity of 0.2 LogMAR.
Conclusions:
Abiotrophia defectiva is an organism that must be taken into consideration in culture-negative cases of post-cataract endophthalmitis. With prompt treatment according to the guidelines good visual outcomes can be achieved.
