Management of postoperative iris cyst: a case report

Abstract Details

Purpose:

To present a case report of  postoperative iris cyst that developed 2 years after extracapsular cataract extraction (ECCE).  We present clinical features and our management of  this rare postoperative complication.

Setting:

In October 2018 68-year old diabetic and cardiac patient presented with symptoms of progressive visual decline in pseudophakic left eye and cystical formation in anterior eye segment at  Department of Ophthalmology Clinical Hospital Center Rijeka, Croatia. Biomicroscopy showed cyst of iris extending from superior quadrants to upper pupilary margin.

Methods:

Our management included complete clinical examination (visual acuity, slit lamp and fundus examination, intraocular pressure) and B-scan. Anterior eye segment changes were fotodocumented.  Our first surgical treatment consisted of Nd:YAG laser cyst rupture with partial removal of the cyst. Because of  slow reccurence of cyst during next  4 months, we performed surgical exicion of cyst with pathohistological  diagnosis.

Results:

Results of our performed surgical treatment were successful with good postoperative visual recovery with minimal inflammatory reaction. Visual acuity improved from preoperative 0,05 to postoperative 0,15 according to Snellen chart.  Pathohistological tissue analysis showed no tumorous or inflammatory changes. Patient will be carefully monitored for following period.

Conclusions:

Basic principle in the management of  large secondary iris cysts is complete excision of proliferative epithelial tissue in cyst wall and the associated epithelial downgrowth, with minimal surgical collateral tissue damage.
