Real World Experience of Goniotomy + phacoemulsification in open angle glaucoma: non-comparative 6 months study

Abstract Details
Purpose:
The purpose of this study was to assess intraocular pressure (IOP) reduction, and reduction in glaucoma medications following goniotomy combined with phacoemulsification (GPE).
Setting:
Ophthalmology Teaching Unit in a large District General Hospital, UK.
Methods:
Prospective 6 month follow up of eyes undergoing GPE (n=30). Single-centre, single surgeon (EA).
Number of glaucoma medications and IOP were recorded at baseline and during post-operative visits through 6 months. There was no drug washout.
Baseline demographics, diagnosis, stage of glaucoma and adverse events were recorded.
Results:
At 6 months, mean IOP was significantly reduced to 13.1+/-2.4 mmHg from baseline 24.2±4.8 mmHg (P<0.001). Mean number of medications was significantly reduced to 0.7±1.4 from 2.4±1.8 (p<0.001).
All cases were open angle glaucoma at moderate stage (mean deviation -6dB to -12dB). F: M=20:10. All cases were completed with good visualisation throughout.
One case of Descemet’s membrane tear without visual compromise, and 5 cases of hyphaema cleared by week 2 post-operatively.
Conclusions:
GPE realised statistically significant reduction of IOP and glaucoma medications at 6 months of follow-up, with no sight-threatening complications.
In GPE cases at 6 months, 90% of eyes experienced IOP reduction of ≥ 20% from baseline and 90% of eyes experienced reduction in the use of at least one ocular hypotensive medication.
