Improvement of ocular presentation documentation in primary care

Abstract Details

Purpose:

Notoriously, medical professionals have low confidence towards ocular problems, and commonly do not know what to look for. This audit aims to assess the documentation of patients diagnosed with conjunctivitis in primary care against the guidelines of what must always be assessed and documented. In Wessex, there is a push to improve primary care management of ocular health. Propose recommendations to bring documentation to target standards (conditional to the results of the former aim)

Setting:

Primary care, group of practices in Dorset, UK

Methods:

Surveys were also completed for a) availability of eye examination equipment to GPs, b) GP attitudes and confidences towards ocular health and conjunctivitis. SystemOne (the electronic health record system used in the group) templates were also assessed for completeness. Flashcards were placed in GP rooms and used as a reminder for red flag documentation in primary care.

Results:

Red flags were documented 100% of the time, except for photophobia, documented at 80% (4 out of 5). Flashcards were shown to be effective at improving documentation for conjunctivitis patients but a longer audit should be performed to ensure this is not due to the chronological proximity to the original audit.

Conclusions:

The results of the mini-re-audit are very encouraging, with huge improvements made to the documentation. flashcards are a good method of reminder for GPs to document red flags in ocular presentations.
