Abstract Details
Purpose:
Corneal hydrops consists of an aqueous infiltration of the cornea after a breach in the descemet membrane and usually complicates keratoconus. The management of acute hydrops has significantly improved during the last years.
Setting:
the Ophthalmology Department of Med VI Hospital in Marrakech, Morocco
Methods:
In the present study, we report a series of 07 patients displaying an acute hydrops on keratoconus from the Ophthalmology Department of Med VI Hospital in Marrakech over a period of 2 years.
Our sample consisted of 3 men and 4 women, and all patients were diagnosed with keratoconus.
The management consisted of a suture of the decemetics breach using Monofil 10-0 (perpendicular to the breach) with air injection into the anterior chamber for 05 patients and SF6 gas for 2 patients.
All our patients received initial medical treatment using lubricants and systemic ocular hypotonizing (acetazolamide per os).
Results:
The average age of our patients was 20.14 years old. 2 patients had allergic conjunctivitis and 1 patient has Down's syndrome. The patients were admitted first in the Emergency Department for ocular pain, photophobia, and decreased visual acuity. In all cases, the acuity on the day of admission was less than 1/10.
Following bilateral complete clinical examination, hydrops complicating a keratoconus diagnosis was retained for all. The regression of the corneal edema started on the first day. The CVAM of glasses was at FC 2m and at 8/10 after scleral lens ingilence.
Conclusions:
The corneal hydrops is a rare complication of keratoconus and the suture of the decemetic breach associated with the injection of gas or air allowed the acceleration of the healing process.
