Keratoconus with stromal scar: which optical correction?

Abstract Details

Purpose:

Keratoconus is a non-inflammatory disease characterized by thinning and bulging of the cornea, usually occurring during adolescence and slowly progressive progression. It causes a decrease in visual acuity because of the importance of irregular astigmatism and the frequent occurrence of corneal opacities. The histological involvement predominates in the Bowman's layer and the corneal stroma. The exact pathogenesis of this condition is still unclear, detected thanks to the development of the videotopographic examinations and that it is important to detect before a refractive surgery this work shows the interest of scleral lenses in the optical correction of the advanced keratoconus

Setting:

Adult ophthalmology department. Hospital 20 août,1953. CHU Ibn Rochd Faculty of Medicine and Pharmacy of Casablanca university hassan 2 casablanca

Methods:

male patient 28 years old without a particular pathological antecedent concept of ocular friction wearer of correction glasses dating from 3 years R: +2.00 (-1.50 to 100 °) L: +2.25 (-2.50 to 170 °) Diagnosed for stage 4 keratoconus in both eyes for 3 months in whom the ophthalmological examination found a paraxial stromal scar in both eyes with visual acuity at 1/10 difficultly

Results:

the patient was non-tolerant to the rigid gas-permeable lenswith scleral lenses the patient found a satisfying visual acuity of 7/10 Prescribed scleral lenses R:       SAG: 5200 Diameter: 16.3 Power: +4.00 bi-toric -0.75 to 90 L:       SAG: 5000 Diameter: 16.3 Power: +5.00 bi toric -1.25 to 160

Conclusions:

Scleral lenses help correct patients with corneal ectasia that is not adaptable or intolerant to LCPG its geometry without contact with the cornea = extended indications to ocular surface alterations to major droughts.  helps to avoid the keratoplasty, to delay it in the evolutionary pathologies  significantly improve the quality of life of wearers.
