Classical and alternative algorithm for the treatment of bacterial keratitis in antibiotic resistance: a case report

Abstract Details

Purpose:

To report a clinical case of Pseudomonas aeruginosa- keratitis and to specify the treatment algorithm in antibiotic resistance.

Setting:

Ophthalmology Department of University Hospital ''Queen Joanna-ISUL'', Sofia, Bulgaria

Methods:

Slit-lamp examination, visual acuity, tonometry, corneal scraping and contact lens for culture and sensitivity report, AS-OCT, fluorescein staining and photography.

Results:

A 42-year-old female with a history of continuous and extended contact lens wear, presented with bacterial keratitis in the right eye. The classical treatment for bacterial keratitis was used before sensitivity report, containing fortified eye drops (Aminoglycosides, Third-generation Cephalosporins, Fourth-generation Fluoroquinolones, Antifungals, Povidone-iodine eye drops, cycloplegycs and lubricating eye drops). The contact lens culture was positive for multidrug resistant Ps. Aeruginosa, sensitive to Meropenem. Ceftriaxone collyr was discontinued due to resistance and replaced by fortified eye drops of Meropenem 50mg/ml. Improvement was observed 48 hours after the first administration of Meropenem eye drops.

Conclusions:

No signs of improvement were observed before administrating Meropenem topically. Fortified drops of Meropenem (50mg/ml) could be used as an addition to the classical treatment of multiresistant and highly aggressive Ps. Aeruginosa- keratitis.
