Purpose:
To report through a medical observation a rare case of an association of intestinal and ocular inflammatory disease
Setting:
adult ophtalmology departement Casablanca
Methods:
Mrs. R ,38 years old, who describes in her ophthalmological antecedents a red eyes with repetition and she’s being followed for a primary sjögren’s syndrome since 05 years, also she describes a repetition of intestinal pain and diarrhea.
on her ophthalmological examination for the concerned eye we found a visual acuity at 0.2, a superficial punctiform keratitis and a temporal corneal perforation measuring 2mm blocked by the iris.The examination of the other eye found a visual acuity at 0.6 with a dry eye.
Results:
the patient benefited from a conjunctival coverage with artificial tears.
An immunological assessment was requested and an ileo-colonoscopy was performed with multiple biopsies. The result was in favor of Crohn's disease.
Conclusions:
Ocular involvement is variable during Crohn's disease. The Ocular involvement is variable and it’s dominated by anterior uveitis.
It could be dangerous and could engage the visual function.
the treatment is based on local and / or general corticosteroid therapy in the benign forms
the collaboration between ophthalmologists, gastroenterologists and internists is required for a good care.
