Comparison of different platform settings of 5 MHz femtosecond laser for FS-DSEK graft preparation: an experimental study

Abstract Details

Purpose:

to evaluate and compare the stromal bed smoothness using different settings of 5 MHz femtosecond laser platform (Femto LDV Z8, Switzerland)  for endothelial approach DSEK graft preparation.

Setting:

S. Fyodorov Eye Microsurgery State Institution, Moscow

Methods:

DSEK grafts were created in 20 ex-vivo porcine eyes using different platform  settings of the Femto LDV Z8 femtosecond laser: Lamellar keratoplasty (n=5), LASIK (n=5) and Modified parameters (n=5). A microkeratome-assisted DSAEK(n=5) served as control group.  The quantitative parameters of the stromal bed smoothness of grafts were the average roughness of surface (Ra) and differences between the tallest peaks to deepest crypts (Rz). These were measured with the 3D-digital microscope KH-8700 (Japan). In addition, laser time of different femtosecond laser settings were compared.

Results:

The  qualitative assessments of stromal bed smoothness  were similar between  the three study groups .The median Ra and Rz parameters  were 0.37(0.31;0.42) and 0.80(0.73;0.92) in the Lamellar keratoplasty group, 0.38(0.33;0.43) and 0.99(0.89;1.04) in the LASIK group and 0.33 (0.28;0.37) and 0.80(0.71;0.94) in the Modified parameters group (p>0.05 for all) There were significant difference of Ra and Rz between the control group 0.26(0.25;0.28) and 0.65(0.62;0.68) to the femtosecond laser groups (p=0.02, p=0.01).   The laser time did differ significantly between Lamellar keratoplasty, LASIK and Modified parameters group and median were 29.3 sec (28.3;29.1), 29(28.1;29.3) and 23.7(23.3;24.3), respectively (p<0.001).

Conclusions:

Different platform settings of 5 MHz femtosecond laser platform for DSEK graft preparation resulted in similar stromal bed smoothness. The use of microkeratome-assisted DSAEK   resulted in smoother stromal bed compared to all femto-DSEK groups. The Modified parameters  platform of 5 MHz femtosecond laser was the quickest platform settings to create the DSEK graft.
