Phototherapeutic keratectomy for granular dystrophy

Abstract Details

Purpose:

Assess the clinical and visual outcome after phototherapeutic keratectomy (PTK) procedure in a patient with granular corneal dystrophy (GCD) without previous treatment.

Setting:

Clinic case

Methods:

PTK was performed for poor visual acuity and discomfort.  The main outcome measures were the visual outcome, clinical course and recurrence of GCD.

Results:

Visual acuity in decimal scale was of 0,2 in the right eye (RE) and 0,15 in the left eye (LE) with refraction of  20º-2.50 -10.00  (RE) and  160º-2.00 -5.50 (LE). Central pakimetry was 566 and 589 microns. The biomicroscopy showed diffuse bilateral anterior stromal deposits in the cornea with no others alterations. PTK procedure was performed using the MEL 80 excimer laser (Carl Zeiss Meditec AG, Jena, Germany). After sixt months of the procedure BCVA was of 0,6 in both eyes, in the exploration the central 3 mm cornea was clear and the pain and discomfort was low.

Conclusions:

In this type of anterior corneal dystrophy the PTK is a safe and effective option to improve the visual outcome and is less aggressive than the keratoplasty (lamelar or penetrating). In addition in the case of recurrence we can repeat the treatment.
