Purpose:
To report a case of a primary Mooren ulcer treated by an amniotic membrane transplantation
Setting:
The patient was seen initially in the emergency department, then admitted in our ophthalmology departement (Adult ophtalmology department, 20 august hospital, Ibn Rochd university Hospital,Casablanca), for 9 days in January 2019.
Methods:
A case of a man who was diagnosed with a primary Mooren ulcer in his right eye and treated by an amniotic membrane transplantation associated with an immunosuppressive therapy.
Results:
The patient is a 56 years old male, arrived to the hospital reporting redness and pain in his right eye. Biomicroscopy showed an oval ulcer with sharp borders in the superior cornea near limbus.
Bacterial cultures were negative, and there was no preceding trauma. The diagnosis of Mooren ulcer was established by elimination of other etiologies for peripheral ulcerative keratitis (PUK).
Initial medical interventions included topical and systemic steroid with autologous serum eye drops. Since the ulcer continued to progress, an immunosuppressive therapy with amniotic membrane transplantation were indicated. The evolution was marked by the healing of the ulcer.
Conclusions:
These ulcers usually respond poorly to conventional therapy, as there is limited knowledge of the pathophysiology of the disease. With further research and understanding of Mooren's ulcer, better treatment options may be available in the future.
