Purpose:
To report a 19-year-old Morrocan boy who developed severe ocular complications including a band keratopathy within Vogt-Koyanagi-Harada (VKH) syndrome. His systemic involvement manifested as vitiligo and hearing loss.
Setting:
department of ophthalmology, HASSAN II University Hospital Centre, SIDI MOHAMED BEN ABDELLAH University, Fez, Morocco
Methods:
Band keratopathy was observed 3 years after the first uveitis attack. The patient was intermittently treated with systemic corticosteroids and occasionally in combination with a transient use of azathioprine. Both eyes showed recurrent and chronic intraocular inflammation. He was operated for cataract with intraocular lens implantation in both eyes at the age of 16 years old. He consecutively had iris capture and membrane formation around the intraocular lens. He underwent peripheral iridotomy with intraocular pressure remaining normal under beta-blocker.
Results:
Currently, he has a uveitis attack in the left eye with the visual acuity that has decreased from 8/10 to 2/10 with an anterior chamber inflammation and hypopyon. For the right eye, Visual acuity was limited to counting fingers and remained steady afterward. Band keratopathy is observed in both eyes but is more important in the right eye.
There is no view of the posterior segment. Ultrasound B-scan shows mild vitreous opacity and flat retina in both eyes.
He underwent systemic corticosteroids with vision and clinical improvement and is now waiting for a corneal transplant for the right eye.
Conclusions:
Band-shaped keratopathy is a relatively uncommon complication in VKH syndrome and mostly occurs in relatively younger patients with a long course of chronic intraocular inflammation. It was frequently associated with complicated cataract and secondary glaucoma and was a marker for poor visual outcome.
