Abstract Details

Purpose:

In morocco, some cataract-blind people are still being treated by traditional healers. Couching is an ancient method of cataract treatment whereby the cataractous lens is dislocated from the visual axis into the vitreous cavity. It is known to be an archaic method which is dangerous to vision, causing many painful and vision-reducing complications.  Through this case we report a Salzmann degeneration after couching therapy of cataract: an unusual complication.

Setting:

We report a case of couching complication seen in Ophthalmology service of Moulay Ismail military hospital Meknes in 2019

Methods:

We describe an unusual clinical case of corneal Salzmann degeneration after couching therapy of cataract

Results:

A 62-year-old man who underwent a complicated couching of cataract in his left eye 3 years ago ( blind eye ) ,presents with a left eye pain and white tumefaction in it's cornea. Slit lampe examination revealed a big white nodule between the corneal epithelium and Bowman’s with marked round prominence on the surface. Anterior Segment Optical Coherence Tomography shows a white, sub-epithelial deposits between Bowman’s layer and the epithelium containning many kystes . we realised a manual removal of the lesion leaving Bowman's layer almost untouched

Conclusions:

Salzmann’s nodular degeneration is a degenerative condition of the cornea occurring worldwide and with specific clinical features. The features in common include bluish-white nodules elevated above the surface of the cornea. Mostly, it has occurred in corneas with a history of keratitisor trauma.  In case of topographically proven growth with decreased surface regularity, documented loss of visual acuity or patients’symptoms manual removal of the nodules followed by repeated excimer laser smoothing using masking fluids is recommended. Occasionally, a simultaneous amniotic membrane transplantation may be advisable.
