Purpose:

The infectious keratitis presents a problem of therapeutic management given the emergence of multi-resistance bacteria .The cross-linking (CXL) is a technique developed to halt the evolution of keratoconus but also has been used in treatment of infectious karatitis. The photoactivated chromophore for infectious keratitis - corneal cross-linking  is a new approach which has demonstrated its effectiveness in the treatment of multiresistance bacteria of infectious keratitis .Our objective was to analyze the time to corneal epithelization and duration of hospitalization with PACK-CXL as a first line treatment in infectious keratitis, and compare it to the standard of care antimicrobial therapy

Setting:

the study is made in ophtalmology department at Hassan II University Hospital , Fez, Morocco.SIDI MOHAMED BEN ABDELLAH University Fez,Morocco.

Methods:

This is a prospective, monocentric, non-randomized and controlled study during 10 month. The inclusion criteria were infiltrates and ulcers up to 2mm in diameter . All patients have received standards treatment of antibiotic therapy .we compare two groups of patients presenting infectious keratitis : group number one have just received treatement by antibiotic therapy, and group number  two have received PACK-CXL with antibiotic therpy after no clinical improvement  in 72 hours. In the PACK-CXL group, epithelial removal around the borders was followed by application of a riboflavin and irradiation with UV-A.

Results:

Thirty nine patients were included in this study, ten in the PACK-CXL group and twenty nine in the medication group. Gram-positive cocci were the most commonly identified pathogens. The most common etiology was the post traumatic ulcer. The corneal re-epithelization time and the duration of hospitalization were observed respectively in the medication groups 7,6 days and 8,4 days versus 12,9 days and 14,1 days in the PACK-CXL group with no significant differences in corneal re-epithelization  time (p=1,22) and also in the duration of hospitalization (p=1,16) .

Conclusions:

The PACK-CXL may be an alternative for treatment for infectious keratitis especially keratitis infectious resistance , even with a tendency for a longer healing.
