Deposits of exfoliative material at endothelium reported by slit lamp photography and anterior segment optical coherence tomography

Purpose:
To study the exfoliative material deposits at endothelium by different types of imaging
Setting:
Vall d’Hebron Hospital. Barcelona. Spain.
Methods:
We present a case of 90-year-old women with pseudoexfoliation syndrome who presented deposits of exfoliative material at the pupillary margin, anterior lens capsule and attached to the endothelium in both eyes. We reported those unusually deposits assessed by slit lamp photography, scheimpflug topography and high definition anterior optical coherence tomography (HDASOCT).
Results:
The imaging techniques used allowed a better description of the morphology, size and location of the endothelial deposits. We found 20 exfoliative deposits at endothelium in her right eye and 18 in her left eye. By HDASOCT we could observe that all of them were located adjacent to the endothelium, but differences in shape and size were noticed. Some presented a round shape while others presented a pedunculated form. Their size ranged from 32x28 to 215x137 microns.
We also measured central corneal thickness obtaining a result of 565/ 571 um for her right and left eye respectively.
Conclusions:
Pseudoexfoliation syndrome is a disease characterized by the accumulation of extracellular material in the anterior segment of the eye, as well as, in other organs. It is well known the presence of this material at the anterior lens capsule and pupillary margin but a few reports are published about the presence of these exfoliative deposits at the endothelium.
We would like to emphasize the importance of imaging to report these cases and to highlight the usefulness of anterior segment optical coherence tomography in particular, to accurately describe the morphology, size and location of these endothelial deposits.
