Corneal perforation on ophthalmic zoster: a case report

Abstract Details
Purpose:
Ophthalmic zoster is a common form, especially in the elderly. It is caused by a
reactivation of varicella zoster virus. Ocular manifestations are dominated by keratitis. Early diagnosis and correct treatment are necessary to avoid complications that compromise visual function such as corneal perforation, that the importance of our observation.
Setting:
Pediatric Ophthalmology department 20 August Hospital, Ibn Rochd University Hospital

Methods:
A 50-year-old man who had been seeing emergency room for right red eye + decreased visual acuity for a significant week, preceded by 3 days before a vesicular rash of the left forehead and eyelid, who had initially consulted with another doctor, he is traited by aciclovir per os at the rate of 2 g / day.
Results:
The examination found a visual acuity with positive light perception, a neurotrophic keratitis with central corneal perforation. The patient underwent blepharoraphy, put on a sufficient dose of aciclovir at a rate of 4g / d for 10 days with local medical treatment(wetting agent + antibiotic prophylaxis). After ablation of the blepharorraphy the patient healed his cornea, but he kept the same visual acuity and a corneal dystrophy for which he is future candidate for a keratoplasty
Conclusions:
Early and adequate antiviral treatment with ophthalmic zoster reduces progression to dreadful complications that threaten visual function such as corneal perforation.
