Abstract Details
Purpose:
the interest of cyanoacrylate glue in the treatment of corneal perforations in patients followed for rheumatoid arthritis.
Collected in the ophthalmology department of Casablanca University Hospital Center.
Setting:
We report the case of a woman followed for rheumatoid arthritis who presented a bilateral sterile paracentral ulcer with corneal perforation in the left eye (1mm)that responded well to treatment with cyanoacrylate and corticosteroids.
Methods:
A 56-year-old woman followed for 4 years in rheumatology for severe rheumatoid arthritis associated with a severe dry syndrome treated with artificial tears . urgently consults for brutal painful bilateral loss of visual acuity with foreign body sensation .
Results:
The ophthalmological examination : visual acuity of 1/10 in the right eye and counting fingres at 0.5 meter in the left eye.
Biomicroscopic examination : bilateral sterile paracentral ulcer with corneal perforation in the left eye (1mm) .
A Schirmer test : impregnation of 1mm paper in five minutes to each eye.
Bacteriological specimens were sterile .
the patient was put on treatment based on :
closure of the perforation by cyanoacrylate glue.
contact lenses .
artificial tears.
oral corticosteroid therapy in decreasing doses.
evolution was marked by complete healing of ulcer and improvement of visual acuity
Conclusions:
Sterile corneal ulcers are rare but potentially serious complications of rheumatoid arthritis .
cyanoacrylate adhesives remain the best choice for small diameter perforations.
Our study illustrates a case that responded well to treatment with cyanoacrylate and corticosteroids.
