Purpose:
To report the feasibility and associated outcomes of a two-piece
microkeratome-assisted mushroom keratoplasty in the treatment of post radial
keratotomy corneal scarring and ectasia.
Setting:
Villa Igea, Hospital, Forlì, Italy
Methods:
Single center, prospective interventional study of all consecutive patients
undergoing mushroom keratoplasty for corneal scarring and/or ectasia after radial
keratotomy (RK) between January 2003 and June 2018. The graft was prepared by
microkeratome and consisted of an anterior lamella of 9mm and a posterior lamella of
6mm. Primary outcomes were refractive astigmatism (RA) and best spectacle-corrected visual acuity (BSCVA). Secondary outcomes were surgical and postoperative
complications, and endothelial cell density (ECD).
Results:
Twelve eyes of 12 patients were included, the mean patient age was 53.4 years
with a mean follow-up of 3.3 years. At final follow up, mean refractive astigmatism
significantly reduced from 4.5 diopters (D) to 2.40 D, and the proportion of eyes with high astigmatism ( above or equal to 4.5D) reduced from 50% to 0% (p<;0.001). Mean BSCVA improved from 0.14 to 0.40 (p<;0.001), in the absence of significant complications.
Conclusions:
Two-piece microkeratome-assisted mushroom keratoplasty is technically
feasible in the treatment of post radial keratotomy scarring and ectasia, with refractive
and visual outcomes not inferior to deep anterior lamellar keratoplasty.
