Topical cyclosporine A as an alternative to topical prednisolone for herpetic stromal keratitis

Abstract Details
Purpose:
To describe the case report of a 52-year-old female with glaucoma and a chronic, recurrent ocular herpes simplex virus disease who presented with a non necrotizing stromal keratitis. She was initially treated with oral antivirals and steroids, and later switched to topical ciclosporine due to elevated ocular pressure.
Setting:
British Hospital, Montevideo, Uruguay.
Methods:
Chart review of a patient who presented to the eye hospital.
Results:
The patient presented with redness, photophobia, tearing and decreased vision in herĀ left eye. The examination revealed a BCVA 20/40, conjunctiva hyperemia, focal cornea opacity and stromal edema; the intraocular pressure was 15 mmHg. She was initially treated with topical prednisolone 1% and oral acyclovir 400 mg/5 times a day. The symptoms and findings gradually improved, however, her intraocular pressure was 25 mmHg after 4 days of treatment. Prednisolone was replaced with topical ciclosporine 1%. Visual acuity, slit-lamp appearance and intraocular pressure significantly improved after 30 days of treatment.
Conclusions:
Herpes simplex virus non necrotizing stromal keratitis can be treated effectively with topical cyclosporine, particularly in cases where the patient is unable to use topical prednisolone.
