Abstract Details
Purpose:
The limbal stem cell deficiency is a disease in which the stem cell and the barrier function of the limbus fail which is expressed by :
- A corneal conjunctivalization associated with a persistent epithelial defects
The chemical/ thermal ocular burn is one of the most common causes of this syndrom
The purpose of this presentation is to to shed the light on the need for a rapid and adequate treatment of such conditions, in order to avoid getting to the transplant, which is the treatment of last resort.
Setting:
We report here the case of a man who suffered a bilateral ocular burn by lime and who was seen in our formation.
Methods:
A 19-year-old patient, with no particular medical history ; admitted in our service after an ocular burn with lime (PH = 12.3).
The initial examination found a visual acuity at ‘hand motion’ in both eyes, with a second degree burn in the right eye and a third degree in the left one ;besides a central corneal ulcer
The conduct adopted : Hospitalisation,abundant eye wash,artificial tears,AIS
autologous serum, therapeutic lens
The evolution was marked by the appearance of a vascular call on 360 ° in both eyes. Subsequently, the patient presented an (LSCD).
The patient was then referred for corneal transplantation.
Results:
The Initial treatment must achieve three main objectives:
- Prevent conjunctival invasion
- Substitute the lack in LSC
- Restore a limbal stromal microenvironment
If the LSCD persists aftere this initial approach a corneal transplantation may be needed
And this was the case of our patient.
Conclusions:
The LSCD may occur following numerous affections of the ocular surface and in particular the ocular burns.
The initial treatment without a transplant may be favorable, but in most cases a corneal transplant is required. Hence the interest of prevention and awareness of the population regarding eye burns.
