Purpose:
Keratoconus in children is a severe and rapidly progressive disease. Children with allergic terrain, rubbing their eyes, with keratoconjunctivitis or trisomy 21, are more often affected. Screening and early diagnosis by corneal topography are essential for any child with recent onset or evolution astigmatism.
The purpose of the case is the sensitize for early diagnosis of children's keratoconus
Setting:
Ophtalmology department, university health center in Rabat, Morocco
Methods:
We report the case of an 8-year-old girl who consulted for bilateral progressive loss vision with visual fog, brutal pain and whitish cornea noticed by the mother who reports chronic friction of eyes with allergy.The examination found in the right eye a central corneal opacity with protrusion and corneal deformation in profile. The slit lamp examination found an hydrops with lower temporal thinning associated with diffuse edema. The left eye was clear, hyperemia and conjunctival papillae was found. A corneal topography was performed; a bilateral keratoconus stage 4 on the right eye and Stage 3 on the left.
Results:
Our patient was hospitalized and put on hypotonic treatment and analgesic by the general way. A crosslinking EPI ON with HYPOTONIC RIBOFLAVINE on the Left eye was realized and a therapeutic lens on the right eye, awaiting a corneal graft.
Conclusions:
Keratoconus in children is a serious and rapidly progressive disease. It is necessary to diagnose it early. An astigmatism of appearance or recent evolution in a child must make suspect a keratoconus and lead to the realization of a corneal topography. The disease can evolve rapidly . he cross-linking of corneal collagen can be proposed to a child with signs of disease progression.The management of amblyopia is essential in a very early form and very asymetric. keratoconus is rarely amblyogenic.
