Corneal wounds in children: prognostic factors and management of irregular corneas

Abstract Details

Purpose:

The aim of the study is to specify the main prognostic factors in corneal wounds in children, the evolution of visual acuity and management of irregular astigmatism and residual corneal scarring, and the important role of lens adaptation in the management .

Setting:

Ophthalmology department -University Hospital Center Mohammed VI -Marrakech ,Morocco

Methods:

Our study is a retrospective study, including 130 children under 15 years old. All hospitalized in our ophthalmology department of university hospital center Mohammed VI -Marrakech for the management of a corneal injury or wound during the period from January 2013 to September 2019. The average duration of surveillance varies from 12 to 48 months. All children received urgent management of the corneal wound and treatment of associated lesions and possible complications  and a treatment of amblyopia. In some cases we also proceeded to a lens adaptation (rigid or scleral) to treat astigmatism and aphakia .

Results:

The average age was 7.5 years ,The initial admission examination had shown visual acuity less than 1/10 in 64.79%,  .For the associated lesions, they were mainly represented by cataracts and scleral wound and aphakia   .The most common causes of low vision in our study were : irregular astigmatism in 32.39% of cases,  axial corneal scar in 33.8% .58 children received optical correction after wire removal,8 children benefited from adaptation by rigid lenses and 5 by scleral lenses and 8 children benefited from transfixing keratoplasty.Regarding the evolution of visual acuity: the final VA was greater than  to 4/10 in 57.69%.

Conclusions:

Corneal wounds in children pose therapeutic problems (associated lesions - risk of amblyopia). The visual prognosis is always threatened, hence the interest of prevention.Rigid and scleral lenses are o useful in the management of irregular cornea and aphakia .Visual prognosis could be also improved through corneal transplantation hence the interest of sensitization for organ donation.
