Unusual corneal trauma caused by the sting of honeybee

Abstract Details

Purpose:

Corneal bee sting is a very rare reason for ophthalmic consultation. It can lead to several complications and sometimes involve the functional prognosis.

Setting:

This clinical case was admitted in the department of ophthalmology, at the University of Mohammed V, in Rabat, Morocco

Methods:

we report a case of a young man who was admitted to emergency for right sided ocular trauma caused by a bee sting while riding a motorcycle in an apiculture region. First examination has shown a corneal stroma infiltration developed around the site of the retained sting causing pain and blurred vision. The stinger was successfully removed. However, after three days the patient accused an exacerbation of the symptoms despite the initiation of anti-inflammatory and antibacterial treatment (ofloxacin 0,3%).

Results:

The examination showed more important corneal infiltration with increasingly descemet folds, corneal edema, and mucopurulent secretions eyelids. The anterior chamber and the fundus microscopy couldn’t be appreciated. The result of culture was negative. We have introduced fortified eye drops, and within 15 days of treatment the visual acuity improved, and corneal infiltration decreased gradually and became clear.

Conclusions:

This case showed us that a serious secondary infection may complicate corneal bee sting that require the use of broad spectrum antibiotics. We also believe that removing the sting is an important part of arsenal therapeutic.
