Bilateral corneal perforation complicating severe dry syndrome

Abstract Details

Purpose:

Corneal perforations are serious complications. Their management depends on the etiology of the perforation and its size. Conjunctival flap is a good alternative in the cases when the amniotic grafts and corneal grafts are not available.

Setting:

Sjogren's syndrome is manifested by a severe dry eye. Ocular complications are sometimes very serious and dramatic.

Methods:

We report the case of a 65-year-old woman followed since 9 years ago for a severe dry eye and treated symptomatically using artificial tears and topical steroids in self-medication. The patient is monophthalmic left eye. Right eye was lost since 3 years because of a spontaneous corneal perforation complicated by a symblepharon. Currently the patient had spontaneous corneal perforation of his left eye. The ophthalmologic examination finds a visual acuity limited to hand motion, a central corneal perforation mesuring 3 mm of diameter with adherent leukoma. The anterior chamber was completly collapsed.

Results:

The patient underwent a conjunctival flap procedure to recover the perforation. The evolution was good and the corneal healing was obtained after 3 weeks. The patient is scheduled for transfixiant corneal transplantation.

Conclusions:

This observation highlights the need for rigorous follow-up and specific treatment of eye dryness in Sjogrën's syndrome to prevent corneal perforation.
