Interface fluid syndrome after Lasik correction of DSAEK-induced ametropia

Purpose:
Descemet-stripping automated endothelial keratoplasty usually modifies the refraction of the operated eye. We report a case where Lasik performed to improve the unaided vision was followed by interface fluid syndrome that did not resolve after one year.
Setting:
Vista Vision Centre, Verona, Italy
Methods:
Case presentation:A 75 year old pseudophakic man with DSAEK in both eyes asked for visual improvement in his right eye. Refraction was +1 sph -5 cyl @45°, corrected visual acuity was 0.4 LogMAR. Femto-Lasik was performed uneventfully in 02/2018.
Results:
One month after surgery evidence of interface fluid syndrome was discovered. An attempt to solve the problem by flap lifting gave no result. Steroid and anti-glaucoma eye drops were prescribed, but the syndrome did not resolve and a central bulla of interface fluid remained. Almost one year after surgery, penetrating keratoplasty is considered.
Conclusions:
Despite previous reports of success, Lasik may cause interface fluid syndrome in DSAEK eyes. Low endothelial cell count of any cause should be considered as a contra-indication for Lasik surgery.
