Sequels of severe vernal keratoconjuncity (VKC): about a case

Abstract Details

Purpose:

reporting a case of complicated vernal keratoconjunctivitis, in order to highlight its management, its gravity and its impact on visual acuity

Setting:

Adult Ophthalmology Service, Hospital August 20, 1953, UHC Ibn Rochd,  Hassan II university ; Medical School of Casablanca ; Morocco

Methods:

A 15 years old patient coming from a uburban area, having as antecedent: pruritus and redness of the eye since childhood having never consulted. Currently consulting for a debilitating photophobia, with lacrimation and severe eye pruritus, associated with a bilateral decrease of visual acuity resounding on his schooling. Right eye: VA was 3/10, and vernal plate occupying the third of the cornea. Left eye: VA was 2/10, vernal plate occupying the half  of the cornea .  conjunctival hyperemia with giant papillae and limbal Trantas nodules, clear crystalline, with diffuse SPK  and neovascularization over 360 °,were found on both eyes .

Results:

The first management of the patient was surgical: scraping the scarifier plate under topical anesthesia combined with local corticosteroid enhanced then ciclosporin eye drops 1 %: 2 drop / day and anti-allergy treatment (antihistamine and mast cell antidegranulant), corneal wetting and healing agents without preservatives, eyewash solutions and an exhaustive allergy assessment. The evolution was favorable with regression of the irritative ocular syndrome ; improvement in visual acuity at 4/10 (right eye) and no improvement in left eye’s acuity; persistence of corneal opacity sequellae with corneal neovascularization.

Conclusions:

VKC is a clinical form of ocular allergy that can induce major visual complications, and a major impact of the life quality of the child and his parents that can cause school dropout , anxiety and behavioral disorders  that requieres a psychological support .
