A case of recurrent fungal corneal ulcer on contact lenses

Abstract Details

Purpose:

Fungal corneal ulcers are a rare but severe condition, their incidence is increasing because of the more common use of contact lenses. The prognosis of keratomycosis has considerably improved thanks to the recent therapeutics

Setting:

20 august hospital, Ibn Rochd University Hospital, Casablanca

Methods:

We report the cas of a 45 years old woman was received in emergency department after presenting for 10 days a red eye, pain, tearing and loss of visual acuity in her left eye self treated with topical antibiotics. The patient reported that she has type 2 diabetes and a high myopia and regularly uses contact lenses since the age of 26

Results:

The patient was initially treated with fortified preparations of antibiotics (Ceftazidim and Vancomycin) with oral antiviral, IOP reducing drops, tear substitutes and ulcer debridement. The endocrinologist recommended insulin injections to help control diabetes with no improvement of the symptoms. The laboratory analysis of the samples isolated klebsiella pneumoniae and candida inconspicua. An intraveinous treatment by voriconazole was started with a notable improvement of symptomatology and clinical signs, the patient was released under oral and local medication. After, 2 weeks the patient presented a recurrence of the symptomatology, she was hospitalized and given intravenous and fortified preparation of voriconazole

Conclusions:

The severe prognosis of keratomycosis can often be improved by a quick and adequate response adapted to the microbiological results
