Corneal collagen cross-linking in the treatment of fungal keratitis: a case report

Purpose:
To report the use of Riboflavin/Ultraviolet light mediated corneal crosslinking (CXL), as a treatment of fungal keratitis not responding to topical, endovenous and intrastromal antimicrobial therapy.
Setting:
Ophtalmology department of a tertiary level university hospital.
Methods:
A 35-year-old healthy female, contact lens user, after ocular trauma with child's nail,
started with pain, redness on the right eye, eyelid swelling and decreased visual acuity (VA) (0.4 decimal). At the slit lamp exploration we found an epithelial corneal defect accompanied by paracentral infiltrate and edema.
Initially was treated with topical antibiotherapy (taking samples before for microbiological culture). A week later Fusarium solani was isolated, adding topical voriconazol. Given the bad evolution, with fluffy infiltrate, thinning and melting of the cornea, endovenous and intrastromal antifungal drugs were established without significant improvement. Finally CXL was performed.
Results:
After riboflavin and ultraviolet A light collagen crosslinking therapy, there was a significant symptomatic improvement, stating an amelioration of visual acuity (0.8 decimal) accompanied by a decrease in the size and density of the infiltrate and a complete epithelialization with a residual corneal scar.
Conclusions:
Corneal collagen crosslinking could be an effective tool in the management of difficult cases of fungal keratitis.
