Purpose:
To discuss the outcomes of a case series of patients treated with Descemet’s Stripping Automated Endothelial Keratoplasty (DSAEK) due to endothelial failure after posterior pars plana vitrectomy and secondary IOL implantation following complicated cataract surgery.
Setting:
Cornea and Ocular Surface Unit. Vall d´Hebron Hospital. Barcelona. Spain.
Methods:
A retrospective case series is presented.
We present a case series of six patients treated with (DSAEK) due to endothelial failure. All patients were diagnosed with endothelial failure after posterior pars plana vitrectomy and secondary IOL implantation following complicated cataract surgery over the course of one year. In all patients IOL was either sutured to the sulcus or fixated to the iris.
We analyzed the rates of primary graft failure and graft dislocation during the first year and compared it with current literature.
Results:
In the case series of six patients we had very high functional results. Two out of six patients (33%) needed a second intervention due to graft dislocation - one resolved with rebubling and the other one with graft sutures. We also registered one primary graft failure (12.5%)
Conclusions:
In unicameral eyes DSAEK is technically challenging due to high risk of graft dislocation, low air pressurisation and postoperative hypotony. There is very little literature available on this subject. We present a case series with excellent functional results that encourage us to perform this kind of keratoplasty in this type of patients
