Topical anti glaucoma treatment and dry eye syndrome: impact on quality of life!

Abstract Details

Purpose:

Glaucoma is a serious eye condition that can lead to blindness. The measure of the quality of life in glaucomatous patients is important because it allows one to evaluate the impact of the pathology and its treatment on the patient's daily life. The purpose of the study is to determine the presence of dry syndrome in patients glaucomatous and analyze the impact on their quality of life.

Setting:

Adult ophtalmology department, 20 august hospital, Ibn Rochd university Hospital,Casablanca

Methods:

This is an observational cross-sectional study.85 patients followed for chronic open-angle glaucoma were included in the study divided into 3 groups according to number of drop of instilled anti glaucomatous eye drops(G1: one drop,G2: 2 drops?G3: 3drops and G4:4drops).Patients with systemic or ocular conditions likely to interfere with the ocular surface condition were excluded. All patients benefited from complete ocular examination with fluorescein test, tear rupture test (TRT) and have answered the Ocular Surface Disease Index(OSDI) and GlauQOL-17 serveysto evaluate the quality of life. Dry eye syndrome was defined as the presence of a superficial punctate keratitis with decreased TRT.

Results:

Group1 included16 patients, group2:34 patients, group3:20 patients,group4:15 patients.In total,70% of G4 patients, 65% of G3 patients and 39% of G2 patients had dry eye syndrome versus 11% G1(p = 0.01).Patients of group1,an average OSDI score of 12.9 ± 13.1, which was significantly more than group2(16.7±17.0, P=0.007), group3 (19.4 ± 18.1;= 0.0001) or group4(19.7±15.0). We note that the average result of quality of life has always been lower in patients in group3 and 4 compared to patients of group1and2.These differences were statistically significant for sections psyche (p=0.05),daily life(p= 0.01) and especially for the dimension"Constraints"(p=0.01)of GlauQOL-17.

Conclusions:

The ocular surface of glaucomatous patients should be evaluated regularly to improve their quality of life and thus ensure better adherence to treatment. The prescription of Hypotonizing eye drops without preservatives is desirable whenever possible.
