Changing indications for penetrating keratoplasty in Kingdom of Bahrain over 20 years period

Purpose:
to determine the recent indications of penetrating keratoplasty in Bahrain and to compare them with the indications in previous time periods
Setting:
This study was done at a tertiary referral centre in Bahrain. the indications for penetrating keratoplasty continue to change with time. Several factors affect this change . Changes in socioeconomic status , development of new technology, eradication of some diseases and emerging of others are among the key reasons.
Methods:
Retrospective study by reviewing case notes who underwent keratoplasty at a secondary care Hospital.from 2012 until 2016. The indications for the surgery were recorded.The results were compared with previous results done in the same institutions during a previous time period 1996 - 2002 and the time period from 2003 until 2011 The change of the trend was observed and analysed.
Results:
99 patients underwent keratoplasty during this time period.
Keratoconus was the commonest indication forming 41% followed by pseudophakic bullous keratopathy 13 %.
Non specific corneal scarring was the third commonest forming 11% of the total cases. Microbial Keratitis formed 10%.Corneal decompenation due to various reasons was 9% Regrafting 7%. Mechanical trauma was 4%. Herpetic keraitis 3 % and Fuchs endothelial dystrophy 1%. During 1996 until 2002, old trachoma used to be the leading indication forming 43% followed by keratoconus32%. Trachoma has then regressed afterwards forming 25% of all cases and keratoconus took the lead from 2002 until 2016.
Conclusions:
Non specific corneal scarring due to old trachoma used to be the leading indication 20 years ago. Because of the improvement in Socioeconomic status in Bahrain, with more health awareness and hygiene, this has regressed. Keratoconus became the leading indication from 2002 onwards.
Pseudophakic bullous keratopathy has become more frequent and became the second commonest indication after 2012 . This is probably because of the change to all phacoemulsification and the nature of the institution being a teaching hospital.
