Abstract Details
Purpose:
describe the clinical, therapeutic and evolutionary aspect of corneal neurotrophic ulcers
Setting:
Ophthalmology Department, Mohammed VI University Hospital, 40000 Marrakech, Morocco.
Methods:
We report a patient with rheumatoid arthritis who presented a neurotrophic corneal ulcer following NSAID use;
A 64 year old patient consulted at a city ophthalmologist 2 weeks ago for a red right eye.
A prescription with NSAID-based treatment has been prescribed, patient used it for both eyes
Results:
Right eye:Visual Acuity: Finger Count at 1m, a pre-perforated Desmetocele, an adherent Leucom , Normal deep chamber , the rest not seen.
Left eye:Visual acuity: 6/10, Corneal ulcer with clean bottom, Normal deep chamber , Regular round pupil. Corneal sensitivity was decreased in both eyes
treatement: Stopping the NSAID, Artificial Tears, autologous serum,daily monitoring
favorable evolution in the left eye
In the right eye :evolution with sequelae including central corneal opacity.
Conclusions:
The diagnosis and management of neurotrophic keratitis is a real challenge.
In front of any chronic corneal ulcer, the corneal sensitivity has to be tested.
Current medical or surgical treatments aim at improving epithelial healing, curbing evolution and preventing potential complications.
