Perforating corneal ulcer complicating ocular rosacea: a case report

Abstract Details

Purpose:

Corneal perforation is a major cause of ocular morbidity and blindness . They constitute the final stage of the evolution of infectious processes and immune pathologies . Corneal perforation following ocular rosacea is a very rare entity. We describe the case of a patient with a perforating corneal ulcer complicating ocular rosacea .

Setting:

A 21-year-old patient without any significant ophthalmological history, who presented to ophthalmic emergencies for redness, pain, photophobia and decreased visual acuity in the left eye with no history of trauma. The visual acuity at admission was 1/10, Slit lamp examination revealed perforating corneal ulcer at 8 o'clock.

Methods:

Examination of the eyelids of both eyes revealed a hyperemia of the palpebral edges with exteriorization through the meibomian orifices of a viscous and trouble liquid at digital pressure. After eliminating the other differential diagnoses; the diagnosis of perforating ulcer complicating ocular rosacea was retained. The patient benefited from an epithelial-coated amniotic membrane graft down the ulcer associated with placement of therapeutic contact lens. An antibiotic treatment based on Cycline has been introduced associated with good eyelid hygiene and artificial tears. The evolution was slow but favorable with closure of the perforation

Results:

The etiology of rosacea is unknown, Cutaneous signs seem to be of vascular origin , while ocular complications reveal an independent mechanism which consists in a primary  meibomian dysfunction. Ocular rosacea can have various ocular signs , Inflammatory recurrences are painful and can even lead to corneal perforation as in our patient .

Conclusions:

This clinical case highlights the existence of potentially severe corneal complications that may compromise the visual prognosis during ocular rosacea.
