Multiple serous retinal detachments following uncomplicated cataract surgery in a patient using tadalafil: a case report

Purpose:
To report a case of multiple serous detachments after cataract surgery
Setting:
A 55-year-old patient presented for cataract surgery at the Athens Eye Center, Athens, Greece for routine cataract surgery.
Methods:
An otherwise healthy patient with LOCS II cataract received phacoemulcification and IOL implantation by an experienced cataract surgeon of the Athens Eye Center. On postoperative day 1 he presented with gradual visual deterioration over the previous evening. His visual acuity had dropped from sc 0,7 straight after surgery to counting fingers, that did not improve with pinhole. Optical coherence tomography showed multiple serous retinal detachments and extensive cystoid macular oedema. Upon questioning the patient disclosed using tadalafil (Cialis) for erectile dysfunction. He was started on dexamethasone 0,1 % and nepafenac drops.
Results:
On the second postoperative day both the cystoid macular oedema and serous detachments had improved significantly and the vision had improved to sc 0,4. A moth later the retinal changes had resolved completely and the vision had improved to sc 0.9. Upon operating the second eye the patient was given prophylactically hourly steroids and a subconjunctival steroid injection. He also paused tadalafil use for a month. The postoperative course was uncomplicated
Conclusions:
Tadalafil may influence the retinal pigment epithelium function similar to sildenafil and cause dysfunction of the blood-retina barrier after cataract surgery.
