Abstract Details

Purpose:

Presentation of a simple surgical treatment protocol for preexisting corneal astigmatism during cataract surgery

Setting:

Hospital Universitario de San Juan de Alicante, Spain

Methods:

Development of an easy visual algorithm for all anterior segment surgeons for the management of corneal astigmatism during routine cataract surgery in a public hospital where toric IOLs are only allowed upon three corneal diopters and femtosecond laser is avaible.

Results:

Right eye: Curved Axis (CA) between 30 and 120º: main 2.2 incision at 180º and: 0.50-1D: secondary 2.75mm incision at CA 1.25-3D: femtosecond arcuate incisions (FAI) Curved Axis between 120-210º: 0.75-1D: two 2.2 opposite paired incisions CA 1.25-1.75D: two 2.75 opposite paired incisions CA 2-3D: main 2.2 incision at 180º+FAI Left eye: Curved Axis between 20 and 80º 0.75-1D: two 2.2 opposite paired incisions CA 1.25-1.50D: two 2.75 opposite paired incisions CA 1.75-3D: main 2.2 incision at 20º+FAI Curved Axis between 80 and 200º: main 2.2 incision at 20º and add: 0.50-1D: one 2.75mm incision CA 1.25-3D: FAI

Conclusions:

The implementation of a simple algorithm for the treatment of astigmatism is advisable for any cataract surgeon. Visual access for all staff in the operating room facilitates the implementation of the guidelines to follow. However, although femtosecond technology is not usually avaiable in public health , the performance of arcuate corneal incisions with laser can be substituted with manual incisonal surgery guided by more traditional nomograms.
