Topography and digital marker assisted IOL implantation: a hospital based study in Pakistani population

Purpose:
To determine outcome of Toric IOL implantation using digital marker with cylinder power and axis determined by the topography
Setting:
All surgeries were performed by single surgeon at the Aga Khan University Karachi Pakistan. All surgeries were performed using Centurion cataract machine, Verion digital marker and AcrySof Toric IOL
Methods:
Prospective analysis of 154 patients implanted with Toric IOL from June 2018 to May 2019. Pre and post op information regarding subjective refraction, autorefraction. topography, IOL biomaster, incision axis, IOL implantation angle were documented on the Excel worksheet. Clinical findings were statistically analyzed using SPSS software version 15.0 (SPSS Inc, Chicago, IL). Means and standard deviations were calculated. Comparisons of the pre and post operative data were performed with the t-tests. A pvalue less than 0.05 was considered statistically significant.
Results:
Average Preop visual acuity was 0.48 and post op visual acuity was 0.09. Average pre op cylinder power was -1.04, while average post op cylinder power was 0.12. IOL misalignment occurred in 2 patients postoperatively requiring realignment.
Conclusions:
Digital marker has shown to improve accuracy when using Toric IOL. The IOL choice is dependant on quality of the data and our study showed use of topography for cylinder power and axis determination, and online Alcon Toric calculator resulted in clinically significant improvement in cylinder reduction. Moreover, AcrySof IOL showed stability post operatively
