Astigmatism of “even” corneas: toricity calculation of trifocal toric IOLs utilizing spectacle refraction

Purpose:
To enhance the refractive outcomes of trifocal IOL based presbyopia correcting surgery of eyes with “no-cylinder” anterior keratometry via toric versions of the lens based on spectacle refractive astigmatism.
Setting:
Outpatient surgery clinic Artoks, Moscow
Methods:
27 eyes of 17 patients undergoing clear lens exchange surgery for presbyopia correction via trifocal IOLs were included in this study. Inclusion criteria were: power difference between main meridians of anterior keratometry no more than 0.25 D, no comorbidities. To correct presumed total corneal against-the-rule astigmatism for all patients toric versions of trifocal IOLs with a cylinder power of 1.0 D in a lens plane were chosen to be implanted. In all cases trifocal toric IOLs were aligned horizontally (0 to 20 or 160 to 179 degree depending on preoperative spectacle refraction).
Results:
Postoperative residual astigmatism of 27 eyes of 17 patients were evaluated. 27 eyes (100%) had a postoperative residual astigmatism within 0.25 D.
Conclusions:
Adjustment of IOL toricity calculation based on preoperative spectacle refraction improved refractive results, with all eyes achieving a manifest refractive astigmatism within 0.25 D.
