Purpose:
The aim of this study is to show the interest of early surgery in children followed for cataract and its benefits on functional prognosis
Setting:
Ophtalmology department Mohamed VI University hospital Marrakech, Marrakech
Methods:
We conducted a retrospective study involving 41 children operated for unilateral congenital cataract between January 2013 and September 2016.
Were excluded from our study children with a history of eye trauma or uveitis. The surgical act consisted on a phacoaspiration with ICP in the bag for all the cases, associated to a posterior rhexis and anterior vitrectomy for children under five. Implant power was calculated according to the SRK II formula with a sub-correction by age . All our children underwent an optical correction with amblyopia treatment.
Results:
The average age of our children was 3.4 years (3 months to 14 years). Strabismus was the main symptom in more than half of our children.27% of them had a unilateral leucocoria. Visual acuity was assessable in 60% of cases, it was less than 1/10 in 65% of the cases. Post-operative complications were an inflammatory reaction in 24% of the cases, transient hypertonia in 7% of the cases, secondary cataract in 28% of the cases. After a mean follow-up of 3 years, final visual acuity went from counting fingers to more than 5/10 in 55% of the cases.
Conclusions:
The precocity of surgery and the quality of amblyopia follow-up and treatment are the best guarantee of a good visual prognosis
