Spontaneous posterior capsular rupture due to poorly controlled diabetes

Abstract Details
Purpose:
To describe and illustrate an unusual case of spontaneous posterior capsular rupture in a 38-year old phakic female who had no history of trauma and hasn't undergone any eye surgery.
Setting:
Illustrated clinical case report. Birmingham and Midland eye Centre.
Methods:
Clinical history with anterior segment photography, B-Scan images and 20MHz UBM images
Results:
A diabetic patient presented to the Eye clinic with a rapid deterioration of vision in her left eye and HbA1c= 146mmol/mol. Visual acuity was 2/60 in right eye and hand movement in left eye.
Four months prior, she had been diagnosed with cataracts and visual acuity was 6/12 in both eyes.
No history of trauma or any other ocular conditions that can be associated with spontaneous posterior capsular rupture. The rupture is evident on slit-lamp and 20MHz UBM images. Opacifications consistent with lens matter in the vitreous are evident on B-Scan ultrasound images. She was managed with a phaco-vitreo-lensectomy.
Conclusions:
To the authors’ knowledge this is the first report of a spontaneous posterior capsular rupture due to lens swelling caused by poor glycaemic control in diabetes.
