Modified slice the pie technique for the removal of complicated cosmetic iris implants

Abstract Details

Purpose:

To describe a modification of the Slice-the-Pie technique for a safer and easier removal of the BrightOcular and NewcolorIris cosmetic iris implants.

Setting:

Shalash LASIK and Eyecare centers, Cairo, Egypt

Methods:

Arjmand et al described a bimanual technique for removal of the NewColoriris implant by cutting it into 3 sections in a slice the pie fashion. The technique involved the use of micro instrumentation with implant rotation and maneuvering inside the eye, A technique which may be challenging with the BrightOcular iris implant due to its triangular footplates which are more more difficult to manipulate. Our technique involves cutting the implant into 5 parts rather than 3 with less intraocular manipulation and no rotation of the implant inside the eye.

Results:

We used this technique in 8 eyes of a 4 patients who presented to us with progressive drop of vision and acute attacks of ocular pain. Mean Best corrected distance visual acuity (BCVA) was found to be 6/24 on Snellen chart. Mean intraocular pressure was 28.6 mmHg. two eyes showed visual field and retinal nerve fibre layer (RNFL) changes. After removal of all implants, one week post operative mean BCVA was found to be 6/9. two eyes were uncontrolled by antiglaucoma eyedrops and required glaucoma surgery.

Conclusions:

Cosmetic iris implants pose a serious risk to intraocular structures which may end in severe ocular morbidity. Removing these implants is mandatory and it is essential that the removal technique should not add more insult to injury. Our technique requires no rotation or excessive manipulation and can be carried out easily and safely on both the NewColorIris and BrightOcular implant designs.
