Purpose:

is to compare refractive and visual outcome of triple surgery (keratoplasty, Cataract extraction and IOL implantation) & Sequential Keratoplasty (two stage surgery, Keratoplasty followed by cataract extraction & secondary implantation) in keratoconus cases

Methods:

This  is  a retrospective  comparative study of  keratoconus patients who underwent triple surgery or sequential surgery performed by the same surgeon ,  The triple procedure group consisted of 20 patients and the sequential group consisted of 20 patients .Both the preoperative and postoperative (one year follow up ) refraction and visual acuity were collected and analyzed

Results:

The mean age of the patients for the triple group was 39.84  and for the sequential group was 37.32 . The sequential group mean follow-up BCVA was 0.58 ± 0.16  while   the mean follow-up BCVA in the triple group was 0.37 ± 0.21 . So there was  a statistically significant difference between the 2 groups. Sequential mean follow up refraction was (0.80 /-2.79 x 65.53 SD= 2.76/1.66/36.89 )  while in Triple  group the mean follow up refraction was (3.42/ -1.80 x 76.58 SD= 3.62/2.56/46.58) .There was a statistically significant difference between the 2 groups.

Conclusions:

The sequential surgery has advantages over the triple surgery in terms of refractive and visual outcome in patients with coexisting cataract and keratoconus .
